Altace (ramipril) is an angiotensin-converting enzyme (ACE) inhibitor prescribed for the management of hypertension, offering a critical line of defense in the prevention of major cardiovascular events. This medication works by relaxing blood vessels, allowing blood to flow more smoothly and the heart to pump more efficiently. It is a cornerstone therapy in modern cardiology, not only for lowering blood pressure but also for providing proven organ-protective benefits, particularly following myocardial infarction and in patients with high cardiovascular risk profiles. Its role extends to slowing the progression of renal disease in hypertensive patients with diabetes, making it a versatile agent in comprehensive patient care.

Features

  • Active ingredient: Ramipril
  • Drug class: Angiotensin-converting enzyme (ACE) inhibitor
  • Available in oral capsule formulations: 1.25 mg, 2.5 mg, 5 mg, 10 mg
  • Once-daily or twice-daily dosing regimens depending on indication and severity
  • Proven efficacy in reducing systolic and diastolic blood pressure
  • Demonstrated benefit in post-myocardial infarction management and heart failure
  • Slows the progression of kidney disease in diabetic patients

Benefits

  • Significantly lowers high blood pressure, reducing strain on the heart and arteries.
  • Lowers the risk of heart attack, stroke, and cardiovascular-related death.
  • Provides renal protective effects, particularly beneficial for diabetic patients.
  • Improves survival rates following a heart attack when used as part of a standard regimen.
  • Can be used as monotherapy or in combination with other antihypertensive agents.
  • Helps manage congestive heart failure symptoms by decreasing cardiac workload.

Common use

Altace is primarily indicated for the treatment of hypertension (high blood pressure). It is also used to reduce the risk of heart attack, stroke, or death from cardiovascular causes in patients aged 55 years or older who are at high risk for these events due to a history of coronary artery disease, stroke, or peripheral vascular disease. Additionally, it is prescribed following a heart attack to improve survival in clinically stable patients and to treat heart failure post-myocardial infarction. Another common use is in diabetic patients to slow the progression of kidney disease.

Dosage and direction

The dosage of Altace must be individualized based on the patient’s medical condition and response to treatment. For hypertension, the usual starting dose is 2.5 mg once daily. The maintenance dose may range from 2.5 mg to 20 mg per day, administered as a single dose or in two divided doses. For cardiovascular risk reduction, the recommended starting dose is 2.5 mg once daily for one week, followed by 5 mg once daily for the next three weeks, then increased as tolerated to 10 mg once daily. For post-myocardial infarction heart failure, start with 2.5 mg twice daily; if hypotension occurs, reduce to 1.25 mg twice daily. Then titrate as tolerated toward a target dose of 5 mg twice daily. Altace may be taken with or without food. Patients should take the medication at the same time each day to maintain consistent blood levels.

Precautions

Before taking Altace, inform your healthcare provider if you have a history of angioedema, kidney disease, liver disease, diabetes, or if you are on a salt-restricted diet or taking potassium supplements. Avoid becoming dehydrated; ensure adequate fluid intake unless otherwise directed. Regular monitoring of blood pressure, kidney function, and potassium levels is essential. Altace can cause injury or death to a developing fetus; discontinue use if pregnancy is detected. It may also increase the risk of hyperkalemia (high potassium levels). Use with caution in patients with aortic stenosis or hypertrophic cardiomyopathy. Surgery or anesthesia may require temporary discontinuation; inform your surgeon or anesthesiologist that you are taking this medication.

Contraindications

Altace is contraindicated in patients with a history of angioedema related to previous ACE inhibitor therapy. It is also contraindicated in patients with hereditary or idiopathic angioedema. Do not co-administer with aliskiren in patients with diabetes. Concomitant use with sacubitril/valsartan is contraindicated due to increased risk of angioedema. Avoid use during pregnancy.

Possible side effect

Common side effects may include dizziness, lightheadedness, tiredness, or a persistent dry cough. These often diminish as your body adjusts to the medication. Less common but more serious side effects can include: fainting, symptoms of high potassium levels (such as muscle weakness, slow/irregular heartbeat), signs of kidney problems (change in the amount of urine), liver problems (nausea/vomiting that doesn’t stop, loss of appetite, stomach/abdominal pain, yellowing eyes/skin, dark urine), and rare cases of angioedema (swelling of the face, lips, tongue, or throat, which can cause trouble breathing).

Drug interaction

Altace may interact with several medications. Notable interactions include: diuretics (may increase risk of hypotension), potassium-sparing diuretics or potassium supplements (increased risk of hyperkalemia), lithium (increased lithium levels), NSAIDs (e.g., ibuprofen, naproxen; may reduce antihypertensive effect and impair kidney function), gold injections (nitritoid reactions), aliskiren (increased risk of renal impairment, hyperkalemia, and hypotension, particularly in diabetics), and sacubitril/valsartan (contraindicated due to angioedema risk). Always inform your doctor of all medications you are taking, including over-the-counter drugs and herbal products.

Missed dose

If you miss a dose of Altace, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up. Consistent daily use is important for this medication to work effectively.

Overdose

Overdose of Altace may lead to severe hypotension (dangerously low blood pressure). Symptoms can include dizziness, lightheadedness, fainting, and shock. Treatment is primarily supportive and symptomatic, focusing on volume expansion with normal saline. Hemodialysis may be considered, as ramipril and its active metabolite are dialyzable. In case of suspected overdose, seek immediate medical attention or contact a poison control center.

Storage

Store Altace capsules at room temperature (between 20°C to 25°C or 68°F to 77°F), in a tightly closed container, and away from light, moisture, and heat. Do not store in the bathroom. Keep all medications out of the reach of children and pets. Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard expired or no longer needed medication through a medicine take-back program.
