Hytrin (terazosin hydrochloride) is an alpha-1 adrenergic blocking agent prescribed primarily for the management of hypertension and symptomatic benign prostatic hyperplasia (BPH). As a selective antagonist of postsynaptic alpha-1 adrenoreceptors, it facilitates smooth muscle relaxation in vascular walls and the prostate, leading to reduced peripheral resistance and improved urinary flow. Its dual therapeutic action makes it a versatile option in clinical practice, offering both cardiovascular and urological benefits under physician supervision.

Features

  • Active ingredient: Terazosin hydrochloride
  • Available in tablet formulations: 1 mg, 2 mg, 5 mg, and 10 mg strengths
  • Selective alpha-1 adrenergic blocker with high bioavailability
  • Administered orally, typically once daily
  • Demonstrated efficacy in clinical trials for both hypertension and BPH

Benefits

  • Effectively lowers blood pressure, reducing the risk of hypertension-related complications
  • Improves urinary flow and reduces symptoms of BPH, such as hesitancy and nocturia
  • Single-agent therapy potential for patients with concurrent hypertension and BPH
  • Generally well-tolerated with a predictable side effect profile when dosed appropriately
  • Supports long-term management with once-daily dosing convenience

Common use

Hytrin is commonly prescribed for the treatment of mild to moderate hypertension, either as monotherapy or in combination with other antihypertensive agents. It is also indicated for the symptomatic treatment of benign prostatic hyperplasia, helping to alleviate obstructive and irritative symptoms such as weak stream, urgency, and frequency. Its use is determined based on individual patient profiles and clinical evaluation.

Dosage and direction

For hypertension, the initial dose is 1 mg at bedtime, which may be increased gradually to achieve the desired blood pressure response, with a typical maintenance dose ranging from 1–5 mg once daily. For BPH, treatment usually starts with 1 mg at bedtime, titrating upward to 2 mg, 5 mg, or 10 mg once daily based on symptom improvement and tolerability. Dosage adjustments should be made under medical supervision, and the full therapeutic effect may take several weeks to manifest.

Precautions

Patients should be cautious when rising from a seated or lying position due to the risk of orthostatic hypotension, especially during initial dosing or dose escalation. Regular monitoring of blood pressure and prostate-specific antigen (PSA) levels is advised. Use with care in patients with renal or hepatic impairment. Caution is warranted in those undergoing cataract surgery due to the potential for intraoperative floppy iris syndrome.

Contraindications

Hytrin is contraindicated in patients with a known hypersensitivity to terazosin or any component of the formulation. It should not be used in combination with other alpha-adrenergic blocking agents due to an increased risk of hypotension. Avoid use in patients with severe hepatic impairment unless the potential benefits outweigh the risks.

Possible side effect

Common side effects may include dizziness, asthenia, nasal congestion, peripheral edema, somnolence, nausea, and palpitations. Syncope, often associated with initial dosing or rapid dose titration, has been reported. Less frequently, patients may experience blurred vision, impotence, or priapism. Most adverse reactions are dose-dependent and may diminish with continued therapy.

Drug interaction

Concomitant use with other antihypertensive agents, phosphodiesterase-5 inhibitors, or erectile dysfunction medications may potentiate hypotensive effects. Nonsteroidal anti-inflammatory drugs (NSAIDs) and sympathomimetics may reduce the efficacy of Hytrin. Caution is advised when combining with CYP3A4 inhibitors, which may increase terazosin plasma concentrations.

Missed dose

If a dose is missed, it should be taken as soon as remembered unless it is nearly time for the next scheduled dose. In that case, the missed dose should be skipped, and the regular dosing schedule resumed. Doubling the dose to compensate for a missed one is not recommended.

Overdose

In cases of overdose, symptoms may include severe hypotension, dizziness, palpitations, and shock. Management involves cardiovascular support, such as keeping the patient in a supine position and administering intravenous fluids. Vasopressors may be used if necessary, with close monitoring of vital signs in a clinical setting.

Storage

Store at room temperature (15–30°C or 59–86°F) in a tightly closed container, protected from light and moisture. Keep out of reach of children and pets. Do not use beyond the expiration date printed on the packaging.

Reviews

Clinical studies and patient reports generally reflect positive outcomes regarding symptom relief in both hypertension and BPH with appropriate use of Hytrin. Many users note improved quality of life, though some report initial dizziness or fatigue. Adherence to prescribed dosing and regular follow-up are emphasized for optimal results.
