Imitrex (sumatriptan) is a prescription medication specifically designed for the acute treatment of migraine headaches with or without aura in adults. It belongs to the class of drugs known as selective serotonin receptor agonists (triptans), which work by constricting blood vessels around the brain and blocking pain pathways. Common use

Imitrex is used for the acute treatment of migraine attacks with or without aura in adults. It is taken at the onset of migraine symptoms for best results. It is not to be used for the prevention of migraines or for other types of headaches, such as tension headaches. Patients should not exceed the maximum recommended dosage in a 24-hour period.

Dosage and direction

Dosage varies by formulation. For subcutaneous injection: A single 6 mg dose is typical; a second injection may be administered at least 1 hour later if symptoms persist or return, but not to exceed 12 mg in 24 hours. For nasal spray: A single spray (5 mg, 10 mg, or 20 mg, depending on strength prescribed) in one nostril; if symptoms persist or return, a second dose may be taken after 2 hours, not to exceed 40 mg in 24 hours. For tablets: A single 25 mg, 50 mg, or 100 mg dose; if symptoms persist or return, a second dose may be taken after 2 hours, not to exceed 200 mg in 24 hours. Always use the lowest effective dose. Administration instructions should be carefully followed per the prescribing information.

Precautions

Do not use if you have uncontrolled hypertension, ischemic heart disease, history of myocardial infarction, coronary artery vasospasm, or other significant heart problems. Use with caution in patients with risk factors for coronary artery disease unless a cardiovascular evaluation provides satisfactory results. May cause chest, jaw, or neck tightness; discontinue if these symptoms are severe or suggest angina. Not recommended for patients with basilar or hemiplegic migraine. Use is contraindicated in patients with peripheral vascular disease, cerebrovascular syndromes, or severe hepatic impairment. May cause dizziness or drowsiness; avoid driving or operating machinery until effects are known.

Contraindications

Imitrex is contraindicated in patients with: ischemic heart disease (e.g., angina pectoris, history of myocardial infarction, silent ischemia); coronary artery vasospasm, including Prinzmetal’s angina; uncontrolled hypertension; peripheral vascular disease; history of stroke or transient ischemic attack; severe hepatic impairment; within 24 hours of administration of another 5-HT1 agonist (e.g., another triptan) or ergotamine-containing medication; hypersensitivity to sumatriptan or any component of the formulation.

Possible side effect

Common side effects may include: injection site reactions (pain, redness), dizziness, drowsiness, flushing, feeling of warmth or cold, nausea, vomiting, fatigue, chest pressure or tightness, neck/jaw/throat tightness. Less common but serious side effects can include: coronary artery vasospasm, myocardial infarction, arrhythmias, cerebral hemorrhage, subarachnoid hemorrhage, stroke, gastrointestinal ischemic events, peripheral vascular ischemia, Raynaud’s syndrome, seizures, serotonin syndrome (especially with concomitant SSRIs/SNRIs), hypertension, anaphylaxis.

Drug interaction

Do not use within 24 hours of ergotamine derivatives (e.g., ergotamine, dihydroergotamine) or other 5-HT1 agonists. Use with MAO-A inhibitors is contraindicated; avoid use within 2 weeks of discontinuing an MAO-A inhibitor. Concurrent use with selective serotonin reuptake inhibitors (SSRIs) or serotonin-norepinephrine reuptake inhibitors (SNRIs) may increase the risk of serotonin syndrome. Use caution with other medications that affect serotonin levels.

Missed dose

Imitrex is taken on an as-needed basis at the onset of a migraine attack. It is not used on a scheduled basis, so the concept of a “missed dose” does not apply. Do not take additional doses to make up for a dose not taken at the start of an attack.

Overdose

Symptoms of overdose may include hypertension, more pronounced versions of common side effects, seizures, tremor, redness of extremities, decreased reflexes, inactivity, cyanosis, respiratory depression, and loss of coordination. In case of suspected overdose, seek immediate medical attention or contact a Poison Control Center. Supportive care should be administered as required.

Storage

Store at room temperature (20°C to 25°C/68°F to 77°F); excursions permitted between 15°C and 30°C (59°F and 86°F). Protect from light and moisture. Do not freeze. Keep all medications out of the reach of children and pets.
