Maxalt (rizatriptan) is a prescription medication specifically designed for the acute treatment of migraine headaches with or without aura in adults. It belongs to the class of drugs known as selective serotonin receptor agonists (triptans), which work by narrowing blood vessels around the brain and reducing substances in the body that can trigger migraine pain, nausea, sensitivity to light and sound. This product card provides a comprehensive, expert overview of its proper use, mechanisms, and important safety information to ensure informed and effective treatment.

Common use
Maxalt is commonly used for the acute treatment of migraine attacks. It is not intended for the prophylaxis or prevention of migraines. Patients are advised to take it at the earliest sign of a migraine, as early intervention is associated with better outcomes. It should not be used to treat common tension headaches or cluster headaches unless specifically directed by a healthcare provider. Use is restricted to adults; safety and efficacy in pediatric patients have not been established.
Dosage and direction
The recommended dose is 5 mg or 10 mg taken as a single dose. If headache returns, a second dose may be taken after at least 2 hours, not to exceed 30 mg in any 24-hour period. For orally disintegrating tablets: peel open the blister pack with dry hands, place tablet on tongue, and allow it to dissolve before swallowing with saliva; no liquid is needed. For conventional tablets: swallow whole with water. Dosage adjustment may be necessary for patients on certain medications or with hepatic impairment. Always follow the specific instructions provided by your prescribing physician.
Precautions
Do not use Maxalt if you have uncontrolled hypertension, history of heart disease, stroke, or TIA. Use with caution in patients with risk factors for coronary artery disease unless cardiovascular evaluation provides satisfactory clearance. May cause dizziness or drowsiness; avoid driving or operating machinery until you know how the medication affects you. Not recommended for use in the elderly or those with severe hepatic impairment. Inform your doctor if you are pregnant, planning to become pregnant, or are breastfeeding.
Contraindications
Maxalt is contraindicated in patients with ischemic heart disease, history of myocardial infarction, or silent ischemia. Do not use in patients with coronary artery vasospasm, including Prinzmetal’s angina. Contraindicated in patients with uncontrolled hypertension, peripheral vascular disease, or basilar or hemiplegic migraine. Should not be used within 24 hours of treatment with another triptan, ergotamine-containing medication, or ergot-type medication. Hypersensitivity to rizatriptan or any component of the formulation is also a contraindication.
Possible side effects
Common side effects may include dizziness, drowsiness, fatigue, nausea, dry mouth, and chest/jaw/neck tightness or pressure. Less frequently, patients may experience palpitations, tachycardia, paresthesia, or flushing. Although rare, serious side effects can include myocardial ischemia, arrhythmias, or cerebral hemorrhage. Seek immediate medical attention if you experience severe chest pain, shortness of breath, sudden or severe abdominal pain, or signs of an allergic reaction such as rash, itching, or swelling.
Drug interaction
Maxalt must not be used within 24 hours of ergotamines or other 5-HT1 agonists. Concurrent use with MAO inhibitors or within 2 weeks of discontinuing an MAO inhibitor is contraindicated. Propranolol may increase rizatriptan plasma concentrations; a 5 mg dose is recommended in patients taking propranolol. SSRIs and SNRIs may increase the risk of serotonin syndrome. Use with caution in patients taking other medications that affect serotonin levels. Always inform your healthcare provider of all prescription, over-the-counter, and herbal products you are using.
Missed dose
Maxalt is taken on an as-needed basis for acute migraine attacks and does not follow a daily dosing schedule. Therefore, the concept of a “missed dose” does not apply. It should only be used at the onset of migraine symptoms. Do not take extra doses to make up for a missed opportunity; adhere to the maximum daily limit of 30 mg within a 24-hour period.
Overdose
In case of overdose, symptoms may include severe dizziness, syncope, drowsiness, vomiting, and cardiovascular effects such as bradycardia or hypertension. There is no specific antidote for rizatriptan overdose. Treatment should consist of supportive measures, including continuous ECG monitoring and vital sign observation. Contact a poison control center or seek emergency medical attention immediately if an overdose is suspected.
Storage
Store at room temperature between 20°C to 25°C (68°F to 77°F), with excursions permitted between 15°C and 30°C (59°F and 86°F). Keep in the original blister packaging to protect from moisture and light. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.

Reviews
Clinical studies and patient reports indicate that Maxalt is effective for many individuals in reducing the severity and duration of migraine attacks. Users frequently note the convenience of the orally disintegrating tablet and the speed of relief.
