Common use
Naprosyn is commonly prescribed for the management of osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, juvenile arthritis, tendinitis, bursitis, and acute gout. It is also effective in treating primary dysmenorrhea and mild to moderate musculoskeletal pain, such as that caused by injuries or overexertion. In lower over-the-counter strengths, it is indicated for temporary relief of minor aches and pains, including headaches, toothaches, backaches, and menstrual cramps, as well as reduction of fever.
Dosage and direction
For rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis, the typical adult dosage is 250 mg to 500 mg twice daily. The maximum daily dose should not exceed 1,500 mg for limited periods, with a usual maintenance dose not exceeding 1,000 mg daily. For acute pain and primary dysmenorrhea, an initial dose of 500 mg may be followed by 250 mg every 6–8 hours as needed, not to exceed 1,250 mg on the first day and 1,000 mg on subsequent days. For over-the-counter use, adults and children 12 years and older may take 220 mg every 8–12 hours, not exceeding 660 mg in 24 hours. Tablets should be taken with a full glass of water, with food or milk to minimize gastrointestinal upset. Do not crush, chew, or break extended-release tablets.
Precautions
Use the lowest effective dose for the shortest duration possible. Patients with a history of gastrointestinal bleeding, ulceration, or perforation should use Naprosyn with extreme caution. Avoid use in patients with preexisting renal or hepatic impairment; periodic monitoring of renal and liver function is advised during long-term therapy. Caution is recommended in elderly patients, who may be more susceptible to adverse effects. Naprosyn may cause fluid retention and edema; use with care in patients with hypertension, cardiac failure, or other conditions predisposing to fluid retention. It may also impair fertility in women; this effect is reversible upon discontinuation.
Contraindications
Naprosyn is contraindicated in patients with known hypersensitivity to naproxen, aspirin, or other NSAIDs. Do not use in patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs. Contraindicated in the setting of coronary artery bypass graft (CABG) surgery. Avoid use in patients with active gastrointestinal bleeding or peptic ulcer disease. Not recommended during the third trimester of pregnancy due to risk of premature closure of the ductus arteriosus. Should not be used in patients with severe renal impairment or in those with severe hepatic failure.
Possible side effect
Common side effects include nausea, epigastric pain, heartburn, dizziness, drowsiness, headache, and mild fluid retention. Less frequently, it may cause constipation, diarrhea, flatulence, vomiting, stomatitis, tinnitus, visual disturbances, and skin rash. Serious side effects, though rare, include gastrointestinal bleeding, perforation, or ulceration; hepatotoxicity; renal papillary necrosis; acute renal failure; aseptic meningitis; Stevens-Johnson syndrome; toxic epidermal necrolysis; anaphylactoid reactions; and exacerbation of asthma. Cardiovascular risk such as myocardial infarction and stroke may increase with prolonged use.
Drug interaction
Naprosyn may increase the toxicity of lithium, methotrexate, cyclosporine, and phenytoin by reducing their renal clearance. Concurrent use with other NSAIDs or corticosteroids increases the risk of gastrointestinal adverse effects. It may reduce the antihypertensive effect of ACE inhibitors, angiotensin II receptor blockers, and beta-blockers. May potentiate the effects of warfarin and other anticoagulants, increasing the risk of bleeding. Diuretics may have reduced efficacy when used with Naprosyn. Concomitant use with selective serotonin reuptake inhibitors (SSRIs) or serotonin-norepinephrine reuptake inhibitors (SNRIs) may elevate the risk of bleeding.
Missed dose
If a dose is missed, take it as soon as remembered, unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume the usual dosing schedule. Do not double the dose to make up for a missed one.
Overdose
Symptoms of overdose may include drowsiness, heartburn, indigestion, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rare cases of metabolic acidosis, apnea, coma, convulsions, and renal failure. In case of suspected overdose, seek immediate medical attention. Management is supportive and may include gastric lavage, activated charcoal, and correction of electrolyte imbalances. Hemodialysis is not effective due to high protein binding.
Storage
Store at room temperature (20–25°C or 68–77°F), in a dry place, protected from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
