Common use
Nexium is commonly prescribed for the treatment of gastroesophageal reflux disease (GERD), including symptoms such as heartburn, regurgitation, and difficulty swallowing. It is also indicated for healing erosive esophagitis, maintaining healing of erosive esophagitis, and reducing the risk of nonsteroidal anti-inflammatory drug (NSAID)-associated gastric ulcers. In combination with amoxicillin and clarithromycin, it is used for Helicobacter pylori eradication to reduce the risk of duodenal ulcer recurrence. Some patients may use Nexium for managing Zollinger-Ellison syndrome or other hypersecretory conditions.
Dosage and direction
For most adults with GERD, the recommended dosage is Nexium 20mg or 40mg once daily for 4 to 8 weeks. For healing erosive esophagitis, 40mg once daily for 4 to 8 weeks is typical, followed by a maintenance dose of 20mg once daily if needed. For H. pylori eradication, Nexium 40mg once daily is used alongside antibiotics for 10 to 14 days. Take each dose at least one hour before a meal, preferably in the morning. Swallow capsules whole; do not crush or chew. For those unable to swallow capsules, the contents can be mixed with applesauce or water for oral administration, or the oral suspension formulation may be used.
Precautions
Patients should inform their healthcare provider of any kidney or liver problems before starting Nexium. Long-term use (more than three years) may lead to vitamin B12 deficiency; monitoring may be necessary. There is a potential risk of bone fractures with prolonged, high-dose therapy. Hypomagnesemia (low magnesium) has been reported rarely with prolonged PPI use; symptoms may include muscle spasms, irregular heartbeat, or seizures. Nexium may mask symptoms of gastric cancer; appropriate diagnostic evaluation should be conducted if concerning symptoms arise. Contraindications
Nexium is contraindicated in patients with known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation. Concomitant use with drugs like atazanavir and nelfinavir may also be contraindicated or require careful monitoring due to interactions that reduce their effectiveness.
Possible side effect
Common side effects may include headache, diarrhea, nausea, abdominal pain, flatulence, constipation, and dry mouth. Less frequently, patients may experience dizziness, rash, or sleep disturbances. Serious but rare side effects include severe allergic reactions (e.g., anaphylaxis), Clostridium difficile-associated diarrhea, acute interstitial nephritis, cutaneous lupus erythematosus, and subacute cutaneous lupus erythematosus. Drug interaction
Nexium may interact with drugs that require gastric acid for absorption, such as ketoconazole, iron salts, and digoxin, potentially reducing their efficacy. It can increase systemic exposure to drugs metabolized by CYP2C19, such as diazepam, citalopram, and imipramine. Concurrent use with clopidogrel may reduce the antiplatelet effect of clopidogrel. Nexium may also affect the metabolism of methotrexate, leading to increased methotrexate levels. Missed dose
If a dose of Nexium is missed, take it as soon as remembered, unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume the usual dosing schedule. Do not double the dose to make up for a missed one. Consistency in dosing is important for optimal symptom control, but occasional missed doses are unlikely to cause significant issues.
Overdose
There is limited experience with Nexium overdose. Reported doses of up to 280mg have been tolerated with nonspecific symptoms. Esomeprazole is not easily removed by dialysis. In case of suspected overdose, seek immediate medical attention or contact a poison control center. Treatment should be symptomatic and supportive, with close monitoring of vital signs and general condition.
Storage
Store Nexium at room temperature (20–25°C or 68–77°F), with excursions permitted between 15–30°C (59–86°F). Keep the container tightly closed and protect from moisture and light.
