Common use

Calcium carbonate is indicated for the prevention and treatment of calcium deficiencies. It is commonly used in conditions such as osteoporosis, osteomalacia, hypocalcemia, and chronic kidney disease-mineral and bone disorder (CKD-MBD). Additionally, it serves as an adjunct therapy in postmenopausal women, elderly patients, and individuals with malabsorption syndromes. It is also utilized as a phosphate binder in renal impairment and as an antacid for symptomatic relief of gastroesophageal reflux.

Dosage and direction

The recommended dietary allowance (RDA) for calcium in adults is 1000–1200 mg/day elemental calcium, depending on age and sex. For supplementation, doses typically range from 500 mg to 2000 mg elemental calcium daily, divided into two or three doses to maximize absorption. Take with meals to enhance bioavailability and reduce gastrointestinal discomfort. Tablets should be swallowed whole with a full glass of water; do not crush or chew. For patients with achlorhydria, consider administration with an acidic beverage. Dosage adjustments are necessary in renal impairment—consult a healthcare provider for individualized regimens.

Precautions

Use with caution in patients with a history of kidney stones, hypercalcemia, or hypophosphatemia. Monitor serum calcium, phosphate, and creatinine levels periodically during long-term therapy. Avoid concomitant use with other calcium-containing products to prevent hypercalcemia. In elderly patients, assess renal function before initiation. Pregnant and breastfeeding women should not exceed the recommended dietary intake without medical supervision. Not intended for use as a sole source of nutrition.

Contraindications

Contraindicated in patients with known hypersensitivity to calcium carbonate or any excipients. Do not use in cases of severe hypercalcemia, hypercalciuria, or calcium-containing renal calculi. Avoid in patients with hypoparathyroidism or sarcoidosis unless under strict medical management. Contraindicated in individuals with ventricular fibrillation or digitalis toxicity due to risk of arrhythmias.

Possible side effect

Common side effects may include constipation, bloating, and flatulence. Less frequently, nausea, vomiting, or abdominal pain may occur. Hypercalcemia may manifest as fatigue, confusion, thirst, polyuria, or muscle weakness. Rare cases of milk-alkali syndrome have been reported with excessive intake. Allergic reactions such as rash or urticaria are uncommon but possible.

Drug interaction

Calcium carbonate may reduce the absorption of tetracyclines, quinolones, bisphosphonates, levothyroxine, and iron supplements—administer these medications at least 2 hours apart. Thiazide diuretics may increase the risk of hypercalcemia. Concomitant use with calcium channel blockers or digoxin may alter drug efficacy and require monitoring. Corticosteroids and anticonvulsants may decrease calcium absorption. Antacids containing aluminum may increase aluminum absorption in renal impairment.

Missed dose

If a dose is missed, take it as soon as remembered unless it is nearly time for the next dose. Do not double the dose to make up for a missed one. Resume the regular dosing schedule. Consistent daily intake is important for maintaining stable calcium levels.

Overdose

Symptoms of overdose include severe hypercalcemia, nausea, vomiting, anorexia, constipation, abdominal pain, and mental status changes. In severe cases, renal impairment, calcification of soft tissues, or cardiac arrhythmias may occur. Treatment involves discontinuation, hydration, and loop diuretics under medical supervision. In life-threatening hypercalcemia, hemodialysis may be necessary.

Storage

Store at room temperature (15–30°C) in a dry place, protected from light and moisture. Keep the container tightly closed. Do not freeze. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
